CYFN’s Regional Health Survey team

CYFN’s Regional Health Survey team will be available in the TKC Health Boardroom November 2, 2015 – 8:30am to 4:30pm to conduct surveys for the third phase of the Regional Health Survey (RHS-P3)
Complete a Survey and you will receive a $15 gift card and be entered for a chance to WIN a prize.
